Training hours for AAPP records can be submitted through the seminar evaluation form OR through a completed blue sheet.   


BLUE SHEET - a form where the attendee self-reports/self-records the specific training hours and classes attended.


 If you need the specific hours and classes you attended for your records and/or your agency, please download the BLUE SHEET prior to attending the seminar.  You can keep track of your hours/classes each day. 

You may submit a completed copy of this document to AAPP for training records via email to [email protected]   Keep a copy of this completed document for your records.
Attendees will have 30 days to submit their Blue Sheet.



ATTENDANCE CERTIFICATE indicates in certificate form that you attended the seminar. This certificate does not have specific hours or classes attended. Please complete the online seminar evaluation sheet to receive your training attendance certificate. 

Attendees will have 30 days to submit the seminar evaluation.
No certificate will be issued unless you submit a seminar evaluation form
within 30 days of the seminar.
You may also find the link on the website under Seminar. It is recommended that you document
your hours on your own during the Seminar to later log on your evaluation. Seminar training certificates 
will be sent only after the submission of your evaluation and will be sent from Certifier.me,
so you may want to check your SPAM folders if you do not receive an email.
